Things do not happen on accident. My mother was very firm on this point, and she was right.

I agree--I need tickboxes, not radio buttons, because "by accident" and "accidentally" are both acceptable to me, but "on accident" isn't. (Maybe a very small child might say it, but not an adult that I'd know.)

I am from the Midwest and the South, but I live in California and both of my parents are Californian.

I think on accident would be wrong. My old teacher Mrs. Hackenheimer would scold me if I'd say anything like that.
